It is important to let us know of any change of name and/or address so that we can notify the local Health Authority. If you leave the area, you will need to register with another doctor. You can do this by completing the relevant section of your medical card and taking the whole card to the new surgery of your choice. You will then be sent a new medical card, by your new Health Authority.

We regularly review our patient lists to ensure patients are within the practice area. If you are living outside the area and need to register with an alternative GP, we will write and inform you of this asking you to register with a GP in your area.
